There will be times of weakness when someone wants to give up, but persevering through those weak moments will only make an individual stronger. It’s important for a recovering addict to remember why they sought help in the first place and to take a minute to breath and appreciate within themselves the progress that has been made.

There was a study done on water. There were two glasses of water, on one glass hateful words like, ugly, worthless, failure, etc. were taped to the outside of the glass, and on the other glass of water encouraging words like, strong, beautiful, treasured, etc.  were taped to the outside. Scientists took microscopic images of the water and noticed that on one glass the images were sharp and jagged and on the other the images were soft and calm. Crazy, right? Well if the human body is made up of 70% water, wouldn’t it be wise to speak positivity over oneself? The second that negative thoughts enter the mind, combat those thoughts. In doing this, there will be a noticeable change within the mind.

Someone who is working through emotions should surround themselves with mentors and positive individuals who will encourage a healthy lifestyle. It’s said that people are the sum of the five individuals they choose to engage with, so for someone fresh out of recovery it’s crucial they choose wisely.

Take a breath. Breaks allow a person to rest and rejuvenate their mind. When the brain can rest and be at peace, take advantage of that time to get excited about the future. People tend to spend their brain power worrying about what can’t be controlled, when instead they could be looking into every aspect of their life through a different lense. Life is meant to be lived. Conquering the mind takes valiant effort, but the great thing is, it’s attainable.

It’s just as important to show character inwardly as it is outwardly. If the recovering addict shows themselves that they can be an honest person even when nobody’s looking, they will not only practice their honesty, but also be able to trust and feel better about themselves.
It’s easy for an individual to speak about the type of person they want to be; but the real challenge is actually following through with it. When an individual’s actions match their beliefs, they have character.

Even the very first beginning of recovery from addiction (the desire for abstinence) shows a belief, and the action behind it gives, maintaining abstinence, the recovering addict an opportunity to grow their character. To further enhance character, the recovering addict must improve the reliability of their words to action in all aspects.

To be more believable to the world around, the recovering addict can always follow through with what they say they are going to do, live their beliefs, and create a list of the character values wanted in the personality and how a person can go about achieving them. Such as, if the trait wanted is honesty, “What would an honest person do in this circumstance?” should be written.

Achieving goals will also help create character. With each new goal conquered a new trait is often gained. For a goal to be attained another trait may be required: confidence is often looked for in job interviews, so to get hired at a job, confidence must be achieved. If the trait wanted is confidence, once the goal is attained they will have confidence in their character.

It’s just as important to show character inwardly as it is outwardly. If the recovering addict shows themselves that they can be an honest person even when nobody’s looking, they will not only practice their honesty, but also be able to trust and feel better about themselves. Once the recovering addict starts to feel better about themselves, other traits will begin to fall into place.

It’s important to re-establish character once in recovery, because when an addict enters recovery, they are creating a new identity. Often in addiction, positive character traits are lost simply by trying to keep up with an addiction such as a once honest person may start stealing to pay for their drugs or alcohol. All lost traits can be regained by continuously practicing the character traits they want to maintain.

Unintended Consequences: The Potential for Opioid Misprescribing Due to Health Care Reform and Parity

The U.S. has 5 percent of the world’s population but consumes 80 percent of the world’s prescription opioids.
The recent Office of National Drug Control Policy (ONDCP) Meeting on Treating Substance Use Disorders addressed access, recovery and the Affordable Care Act, and focusing on the entry of previously uninsured substance abusers into the medical system.

Estimated that approximately 10 million new patients have been added to the health care rolls in 2014; about 10 percent of them are estimated to be substance abusers. One of the unintended consequences of the increased access to the medical system by substance abusers will likely be inadvertent mis-prescribing of opioids by physicians unfamiliar with addiction assessment. This will escalate the legal debate on how best to respond to the growing drug problem in the U.S., which is surfacing in many different arenas.

The U.S. has 5 percent of the world’s population but consumes 80 percent of the world’s prescription opioids. The hotspots for the spread of the drug epidemic are multiple across the U.S., surfacing in areas where pain medications are prescribed excessively and patients transition to low cost, high potency heroin. Preventing prescription opioid deaths has become a public health priority (Jones et al. 2014), and there is a trend toward upscheduling of specific narcotics, e.g. moving hydrocodone to Schedule II, in order to lessen their use. This provides only a limited solution; improved education and training of medical practitioners on the complexities of pain and addiction is also necessary.

Several factors contribute to this rise in opioid prescribing. Many more opioids are available for treating pain than have ever been previously available. More opioids are being prescribed for long-term pain rather than for only acute pain. In combination with the new medications, and as awareness of pain’s impact on an individual’s quality of life and ability to function has grown, growing numbers of physicians are prescribing opioid medications. These drugs are more commonly found in home medicine cabinets than previously at hand to curious adolescents eager to try the “next high”. The increase in consumer marketing and access to the Internet has made many more individuals aware of these powerful drugs, and a great deal of information is available about them, how to obtain them, and how to use them.

However, a new source of opioid use will soon become apparent, as large numbers of formerly uninsured substance abusers enter the medical system as a result of the Patient Protection and Affordable Care Act (PPACA, aka “Obamacare”). The PPACA mandates that all insureds have a medical “home,” based upon a primary care provider (PCP). Many, if not most, PCPs have little experience with substance abusers and are understandably naïve about the effects of opioids when used in combination with many other medications, the guile and wiles used by many substance abusers, and the importance of screening tools, e.g., CAGE, SOAPP, ORT, DAST-10, to determine quickly a patient’s likelihood of substance use/misuse/abuse.

These clinicians will find the educational resources of the American Society of Addiction Medicine and their local addiction medicine organizations invaluable in avoiding the “4 Ds” of misprescribing (Wesson & Smith 1990):

Dated: not current with prescribing practices or current drug use/misuse patterns

Duped: deceived and/or manipulated into prescribing at variance with accepted medical standards

Dishonest: subverts medical practice for personal financial gain

Disabled: the clinician’s own impairment hinders exercise of optimal judgment

A critical element in the avoidance of over-prescribing is obtaining a family history of substance misuse/abuse. Brief screenings can help to identify individuals who should be prescribed opioids with caution. A sampling of SBIRT, or Structured Brief Intervention and Referral to Treatment, screening tools can be found at www.integration.samhsa.gov/clinical-practice/sbirt  or through a Google search. SAMHSA also offers an “Opioid Overdose Prevention Toolkit” with information for prescribers, patients, families, first responders, and community members. State prescription drug monitoring programs, such as CURES in California, can alert physicians whether a patient is gaming the system.

Prescription guidelines from state medical boards and societies are also available. Additionally, the SCOPE of Pain program at Boston University offers several resources www.scopeofpain.com. In the future, genetic testing will more clearly identify those susceptible to substance misuse.

As increasing numbers of patients with substance misuse issues enter the medical system, primary care providers must have additional education, training and information to appropriately treat the unique needs of those in recovery and at risk for misusing opioids.

Dr. Smith chronicles the rise of the Free Clinic movement that sprung from rampant substance abuse in the Haight Ashbury district of San Francisco during the Summer of Love in 1967.

He is a Fellow and Past President of the American Society of Addiction Medicine, Past President of the California Society of Addiction Medicine, Past Medical Director for the California State Department of Alcohol and Drug Programs, and Past Medical Director for the California Collaborative Center for Substance Abuse Policy Research. Dr. Smith is the original Founder of the Haight Ashbury Free Clinics of San Francisco and has been honored as one of the “Best Doctors in America”. Dr. Smith is also the Founder and Publisher of The Journal of Psychoactive Drugs and co-editor of The International Addiction Infoline Newsletter. In addition, he has authored or co-authored 26 books, written over 340 journal articles, edited 28 journals, and has been the technical consultant for 28 drug abuse-related films.

Inspired by Ken Kesey and the Merry Pranksters described in Tom Wolfe’s “The Electric Kool Aid Acid Test”, and their house band, The Grateful Dead, all fueled by “Owsley Acid” synthesized in the San Francisco Bay Area, large numbers of young people flocked to the Haight Ashbury with a philosophy of “Better Living through Chemistry”. Many now well-known musicians lived in the Haight Ashbury, such as The Grateful Dead, Janis Joplin, The Jefferson Airplane, Country Joe McDonald and members of Crosby, Stills and Nash. This philosophy peaked in the 1967 media frenzy with the Summer of Love, which generated world-wide attention (Brokaw, 2007).

What changed? Was it “Turn on, Tune in, & Dropout” as Timothy Leary stated with the beginning of the psychedelic drug revolution, or “Turn on, Boot up, Jack In” as Steve Jobs described the beginning of the personal computer revolution (Isaacson, 2011)? Or did the dream become a nightmare with the beginning of the speed epidemic which destroyed the Haight Ashbury in 1968 and continues to devastate both the Central Valley and rural communities across the country (Edwards, 2011; Owen, 2007a, 2007b; Smith, 1969b; Smith & Luce, 1971)?

Many of those using drugs lost or saw a decline in their capacity for taking care of themselves as the summer of 1967 drew to a close. Drug use quickly became drug abuse. The Summer of Love all too soon disintegrated into a dark period characterized by sexual predators and dealers of more toxic drugs like amphetamines. Like gravity, what goes up must come down; escapism is only temporary and reality is permanent. Some soldiers returning from Vietnam were addicted to heroin because it was so easy to get overseas and mitigated the horrors of the war they were fighting. They had passed through the neighborhood on their way to Vietnam and having found it attractive with the availability of sex and drugs, they returned.

The Haight Ashbury neighborhood went into a severe decline. Property values plunged, and most of the storefronts on Haight Street were boarded up until the mid-70s. People were afraid to walk the street.

The history of this turbulent period is still being written. In San Francisco and the Santa Clara Valley during the late 1960s, various culture currents flowed together. There was the nascent hacker subculture along with quasi-academics researching the effects of LSD on expanding human potential, and Ken Kesey, whose Merry Pranksters celebrated LSD with music and light shows featuring the band that became known as the Grateful Dead. In the Haight Ashbury, the hippie movement was in full swing, and in Berkeley, there was the anti-war free speech movement. This fusion of flower power and processor power prompted Steve Jobs, co-founder of Apple Computer and a product of the psychedelic 1960s, to comment, “There was something going on, the best music with the Grateful Dead, Jefferson Airplane, Joan Baez and all the integrated circuit.”(Isaacson, 2011).

Experimentation with and use of drugs became more acceptable to a wider, more middle-class segment of the population, whereas earlier drug use – other than alcohol and nicotine – was perceived as having criminal associations. People today are more willing to experiment with drugs than they were previously. Certainly marijuana is more tolerated by more of society. There’s increasing acknowledgement that drugs like LSD enhance creativity and the ability to conjure abstract relationships, and that their use actually enhanced development of the Internet. The drug culture is well documented in many biographies. Drugs opened “doors of perception” but abuse slammed them shut.

The people who invented the 21st Century digital revolution were pot-smoking, acid-dropping, sandal-wearing, long-haired hippies from the West Coast. Like Steve Jobs, they were influenced by Kesey and Stewart Brand of the Whole Earth Catalog and saw things differently, Together they produced the Trips Festival in 1966, creating a sound and light presentation completely unlike anything attendees had ever experienced – especially after they’d ingested the LSD-spiked vats of Kool-Aid freely offered to them. Most of the hippie generation, on the other hand, scorned computers as the embodiment of centralized control, but a tiny contingent, later called the hackers, embraced them and set about transforming them into tools of liberation (Isaacson, 2011).

One of the many social movements that came out of that psychedelic era was the Free Clinic Movement, beginning with the founding of the Haight Ashbury Free Medical Clinic on June 7, 1967 during the peak of the Summer of Love. Founded on the philosophy that “healthcare is a right, not a privilege”, its organizers were motivated by the same liberation and civil rights principles as the other progressive, counter-establishment movements of the time. The Clinic was also a response to the growing drug epidemic, which began with pot and LSD, then devolved to speed, which today continues to devastate the San Joaquin Valley and heroin abuse. Today at North Bay Recovery Center we’re seeing a rise in patients misusing prescription opioids such as Oxycontin. Part of the original Clinic philosophy was that addiction is a disease and the addict deserves the right to treatment. This was the beginning of the medical specialty of Addiction Medicine, which is now a Board Certified specialty, and the founding of the Journal of Psychoactive Drugs, the oldest scientific addiction journal in the United States.

How to Recognize Relapse before it Happens

If it happens, it is not something to be ashamed of because the statistic shows that 80% of addicts will relapse in their first year of recovery. The truth is, the key to staying in recovery and out of relapse is to be able to recognize the signs of relapse.
Relapse is an unfortunate but real possibility of anyone who enters recovery.

Relapsing Emotionally

Relapsing emotionally includes any feelings that bring about the beginnings of a relapse. Signs of that someone is relapsing emotionally include unnecessary anxiety, defensiveness, anger and unusual mood swings. Isolation can also be signs of an emotional relapse which can include no longer being active in a recovery program such as 12 step meetings, resisting invitations to go out with family and friends, and finally not seeking help once the emotional relapse is recognized. The good news is, this beginning stage of relapse is the one the recovering addict is most likely to notice right away, thus making it the easiest to come back from at any point.

Thinking and Planning the Relapse

Thinking about relapsing can perhaps be the most uncomfortable because the addict is constantly thinking about using more as it progresses. What starts as just daydreaming about using days, making them sound better than they actually were (often called “glamorizing”) will quickly turn in to literally picking a day and time to do it. This also includes spending time with old using friends and going to old places where they used. Because the addict is now planning the relapse, it is harder to come back from as it becomes closer to reality.

The Relapse

The relapse is when the alcoholic is actually at the store purchasing the alcohol, brings it home and drinks it. It is not impossible to come back from this, but it does take being honest with others about the relapse and not living in denial.

It is possible to avoid going into relapse mode. Maintaining attendance at 12 step groups, taking care of oneself, and keeping in mind the importance of abstinence will all help to avoid feeling, thinking, and acting out a relapse. The most important thing to do if any of these signs are recognized to tell someone whether it be a recovery group, a sponsor, or a therapist. Breaking the silence of the desire to use will help to stop it.
